Filling up of Application Form

 

Online Submission of Form will be made available from 15th January 2010 to 21st  February, 2010 (extended)

 

1.      The email address of the student will be the login name and new password has to be created for submission of the STS application . A unique ID/number will be generated by the system which must be stored or remembered or noted down  by the student for future reference.

2.      There are 4 sections of the STS 2010 application to be filled ONLINE.

A. Enclosures/ Attachments

B. Particulars of Student

C. Particulars of Guide

D. STS Project Proposal 

  1. Attachments: The enclosures may be attached in pdf, word, or JPEG format.
    1. Compulsory attachment: The most important among these is the ‘Attestation Form’ as per format provided (download)   that has to be filled in and signed by the student, Guide, Head of the Department and the Head of the Institution/ Principal/ Dean of the medical college. Once the attestation form is signed by all, the student must scan it and send it to the Council as an attachment with the application. Application will not be accepted unless accompanied with Attestation Form. The student should download the attestation form paste passport size photograph in the attestation form and get it attested before submitting a scanned copy (in jpg, word (.doc) or pdf  format) of the form to the Council. 
        
    2. Optional Enclosures: The other enclosures are optional and may be also sent with the application as separate attachments (word (.doc)  file, PDF or jpg file). These may include:  institutional ethics committee approval certificate (IEC) or an Institutional animal ethics committee certificate (IAEC) in case approval is obtained. In case approval is not received the student may mention the reasons for the same. In case student has applied and is awaited the same may be informed. In addition if there are plans to use an Informed consent form or any case study form or questionnaire the same may be also enclosed. 
      The student may include the case report form, informed consent form or any other document also as part of project proposal or can submit them separately as attachments. The student must also mention if IEC/IAEC approval has been obtained or is being taken. In case the student had participated in STS 2009 and was unable to complete the project last year he/ she may mention that re-application is being made on the same topic under the same Guide.
  1. Particulars of Students: The student must provide full name, Sex, Medical college, Course and year, contact details. Kindly give the complete addresses in the application form on which stipend can be received from the Council during 2010-11.  Changes in address at a later date will not be allowed. The student should also provide their contact telephone No./Mobile/email to facilitate communication.
  1. Particulars of Guide: Complete details including their Name, Designation, Department, Medical College , Contact details to be provided. The Guide should ensure that she/he selects a suitable brief topic for this studentship, which can be completed within 2 months without asking for extension. The guide may forward the application of only ONE student each year for STS program.
  1. Project Proposal for STS: This should be prepared in brief with the following sections-Title (upto 25 words), Introduction/Background (upto 150 words), Objectives (upto 100 words), Methodology (upto 750 words), Implications/Expected Outcome (upto 75 words), Bibliography (upto 5 references) of the study. The project should describe a brief work plan of proposed research giving a short introduction, objectives, materials and also clearly stating the sample size and appropriate statistical methodology, which will be used along with implications of the research study.
